What are the statutory forms under EPF?

Employee Registration and Nomination Forms.

  • Form 2: Used for declaration and nomination under the Employees’ Provident Fund and Pension Scheme.
  • Form 11: Declaration form for new joinees to declare existing UAN and previous employment details.
  • These forms are mandatory during joining and maintained in employer records.
  • Accurate submission ensures smooth UAN mapping and nomination benefits.
  • Employees must update these forms upon changes in family or marital status.

Claim and Withdrawal Forms.

  • Form 19: For final settlement of EPF account after retirement or resignation.
  • Form 10C: Used to claim withdrawal benefit or scheme certificate under the Pension Scheme.
  • Form 31: For partial withdrawal of EPF for purposes like illness, education, or housing.
  • Form 10D: Required for monthly pension claim after completing eligible service.
  • These forms are submitted with KYC documents and employer verification if offline.

Monthly and Annual Return Forms.

  • Form 5: Details of new employees eligible for EPF, submitted monthly.
  • Form 10: Details of employees who have left service during the month.
  • Form 12A: Monthly statement showing details of EPF contributions paid.
  • Form 3A: Annual contribution card for each member showing month-wise deposit.
  • Form 6A: Consolidated annual statement of contributions for all employees.

Transfer and Administrative Forms.

  • Form 13: Used to transfer EPF balance from previous employer to current employer.
  • It consolidates old accounts under a single UAN.
  • This form ensures continuity of service and accumulation of benefits.
  • Both employer and employee details must be accurately filled.
  • Submission is needed when changing jobs within EPF-covered establishments.

Other Essential Forms.

  • Form 5IF: Used to claim Employees’ Deposit Linked Insurance (EDLI) benefits.
  • Form 20: Claim form for EPF settlement in case of a member’s demise.
  • Form 9: Enrolment form submitted by the employer for each new member.
  • Inspection Book: Maintained by employers for inspection by EPFO officers.
