- Sickness must be certified by an ESIC-authorized medical officer or doctor.
- The benefit is payable only for certified days of illness.
- The insured person should not be receiving wages during the leave period.
Visit an ESIC dispensary or hospital
- The insured person must report to the nearest ESIC dispensary or designated medical facility.

- The doctor will examine and issue a medical certificate or “sickness certificate”.
- This certificate specifies the number of days the employee is unfit to work.
- The certificate must be preserved and submitted for claiming benefits.
Submit Form 9 and medical certificate
- Fill out Form 9 (claim form for sickness benefit) with the insured person’s details.
- Attach the medical certificate issued by the ESIC doctor.
- Submit both documents to the nearest ESIC branch office within the prescribed time.
- The documents may also be submitted through the employer.
- Multiple certificates must be submitted if the illness period is extended.
Await verification and approval
- ESIC will verify the claim based on the contribution status and medical advice.
- If approved, the sickness benefit is processed and sanctioned.
- Any discrepancies or missing documents may lead to delays or rejections.
- The employee may be called for a follow-up or verification visit.
- Notifications of approval or rejection may be shared through post or in person.
Receive benefit payment
- Once approved, the benefit is disbursed directly into the employee’s registered bank account.
- The sickness benefit is paid at 70% of average daily wages for the certified period.
- Payment is typically made in bi-weekly installments for long-term sickness.
- Retain bank receipts and acknowledgment for future reference.
- Continue submitting follow-up certificates if the illness persists.
